Many people in Austria begin their day with a cup of coffee long before they sit down for breakfast. In moderate amounts, the drink is considered beneficial: studies link it to a stimulated metabolism and a reduced risk of cardiovascular disease and certain cancers. But drinking it on an empty stomach doesn’t work equally well for everyone.
The Body’s Reaction
According to U.S. nutrition expert Anthony DiMarino of the Cleveland Clinic, the digestive system can handle coffee without food. Still, some people experience side effects. When caffeine enters the bloodstream more quickly, symptoms such as heart palpitations, trembling, or nervousness may occur.
Coffee also stimulates stomach acid production. This doesn’t cause ulcers, but it can worsen acid reflux or heartburn in those who are prone to it.
Stress and Blood Sugar
A small British study suggests that drinking coffee before breakfast can increase the body’s blood sugar response to the meal by up to 50 percent. That means the body may react more strongly to carbohydrates afterward.
Caffeine also boosts the morning rise of cortisol, the stress hormone. Higher cortisol levels can make the body more sensitive to stress throughout the day.
Another well-known effect: coffee often stimulates the bowels. Certain acids in the drink activate digestion, making it a natural “bathroom booster.”
A Gentler Start to the Day
There is no universally perfect time for coffee, DiMarino says. But people with a sensitive stomach may benefit from drinking it with breakfast rather than before it. Foods rich in fiber—such as oats or whole‑grain bread—and protein sources like yogurt, eggs, or cottage cheese help slow caffeine absorption and protect the stomach lining.
Coffee on an empty stomach isn’t dangerous for most people, but it can trigger discomfort in some. Those who notice jitters, reflux, or heightened stress may want to shift their morning cup to accompany food instead.
